Institutional Memory

Institutional Memory describes how a system retains the knowledge, reasoning, relationships, decisions, and structural logic that allow it to function coherently over time. In human systems, coherence is difficult to sustain when important context disappears with turnover, leadership changes, undocumented decisions, or fragmented communication. Memory allows a system to understand why its structures exist, what they are meant to preserve, and where past patterns of failure or alignment have occurred.

When institutional memory is weak, systems repeatedly rediscover the same problems. People are asked to rebuild context, explain history, or absorb preventable confusion because the system has not retained what it already learned. This creates fatigue and drift.
